When Can Babies Start Cow's Milk? Right Age for Cow, Buffalo & Goat Milk Explained

Dr. Sandip Gupta

A pediatrician answers one of the most commonly asked parent questions: if the mother's milk is not enough and the baby stays hungry and keeps crying, can we give cow's milk to a newborn or small baby — and what is the right age to start cow, buffalo or goat milk (all animal milk). Covered in this video: Why mother's milk is best It is easy to digest, passes the mother's immunity to the baby, and babies on breast milk have fewer problems with allergy and less risk of diabetes or obesity later. Why cow's milk cannot be given as a drink under 1 year Protein is too high: cow's milk has about 3.3–3.4 g protein per 100 ml versus 1 g per 100 ml in mother's milk. This high concentration cannot be digested by a baby's intestines and can cause many problems. Cow's milk is high in casein while mother's milk is higher in whey protein. Casein is harder to digest and causes gas; whey digests easily. Sodium is very high in cow's milk. A baby under 1 year has an immature kidney that has to excrete this sodium, putting stress on the kidneys. Iron and vitamin C are not present in adequate amounts, so repeatedly giving cow's milk under 1 year risks iron and vitamin C deficiency. What to do if mother's milk is not enough First, try techniques to increase mother's milk supply (a separate video covers this). If it is still not enough, a little formula milk can be used until the mother's supply improves (a separate detailed video covers formula). Why formula is recommended over cow's milk even though formula is made from cow's milk: the composition is heavily modified to come close to mother's milk. Indian formulas have about 1.4 g protein per 100 ml, compared to 1 g in mother's milk, 3.3–3.4 g in cow's milk and more than 4.4 g in buffalo milk. Sodium is also brought close to mother's milk levels, along with many other components. The doctor states clearly he is not promoting any formula milk — mother's milk is best, and exclusive breastfeeding for 6 months is what should be tried for. Age guidance Under 6 months: only mother's milk. If it is short or unavailable, formula milk. Cow's milk is not safe under 6 months. Cow's milk as a drink can be started after 1 year, when the intestines have matured enough to digest it and the kidneys can handle the sodium load. Above 6 months (past 180 days), WHO and Indian authority guidelines allow cow's milk cooked into cereals — rice, poha, oats, dalia, barley. Cooking it with a cereal mixes in starch and other things, which reduces the pure cow's milk protein and sodium problem. This is not the ideal situation, but where affordability is an issue and formula is not possible and mother's milk is unavailable, cow's milk can be used to cook kheer or halwa with cereals, since the baby is not drinking pure cow's milk. Discuss with your pediatrician or child specialist first. After 6 months are complete, cow's milk products — curd, yogurt, cheese, paneer — can be given, because fermentation or bacteria pre-digest the protein, and given in small amounts as solid food they provide protein without creating problems. Buffalo and goat milk are also animal milk with protein far higher than mother's milk, and they cause all the same problems as cow's milk, so they too should be started only after 1 year of age. The video ends by noting a follow-up video will cover precautions when starting cow's milk, whether to give packet milk or milk from the local milkman, and how much cow's milk per day to give.
