Everything About Formula Milk — Sugar, Safety, Preparation and Common Myths

A podcast episode answering the most common formula milk questions and myths parents ask about. How to choose the "best" formula milk. There is no single best formula — every company's product is a bit different, like different car models. Judge suitability by three things: digestion (no excess gas, vomiting, loose motion or hard stool), proper weight gain, and affordability for you to continue consistently. Always match the correct stage to age — Stage 1 (0–6 months), Stage 2 (6–12 months), Stage 3 (12–18 months), Stage 4 (18–24 months). Try one formula, check that it suits the baby, and stick with it rather than experimenting repeatedly; only change (on a doctor's advice) if there's regurgitation, loose motion or hard stool. For colicky babies, a formula with a higher whey percentage (whey digests more easily than casein) may be recommended by your doctor. Is the sugar in formula milk harmful? This is a major source of confusion. The word "sugar" on a label covers many carbohydrates — lactose, maltose, fructose, sucrose — not just refined/table sugar (sucrose), which is genuinely unsafe for children under 2. On formula labels you'll see "total sugar" and "added sugar." Total sugar includes natural sugar (lactose, naturally present when cow milk is converted to formula) plus any added sugar (added to meet a baby's carbohydrate needs, since formula is adjusted from cow milk composition). If the added sugar is entirely or mostly lactose — the same natural sugar present in both breast milk and cow milk — there is no need to worry, even if the total sugar figure looks like 35–45 grams; lactose is a natural carbohydrate that provides energy and supports growth. The concern would only be justified if the added sugar were sucrose. Why formula (made from cow milk) is considered safer than plain cow milk under 6 months. Formula is genuinely made from cow milk — that's true — but its composition is deliberately adjusted to resemble breast milk. Cow milk protein content (~3g) is much higher than breast milk (~1g), and the type of protein differs: breast milk is about 60% whey/40% casein, while cow milk is about 80% casein/20% whey — casein is harder for babies to digest. Formula companies adjust this ratio toward breast milk's proportions (roughly 60% whey/40% casein), fortify with extra iron (since cow milk is naturally lower in iron), and reduce the sodium level (cow milk's sodium, appropriate for a calf, is too high for a human infant), plus other minor adjustments. Even with these changes, formula is still not better than breast milk — always prefer breast milk if it's available. Can formula cause gas, allergy or infection risk? Gas and allergy can happen with any formula in some babies, just as they can with breast milk — it varies by baby, so you may need to try to see what suits your child. Infection risk comes not from the formula itself but from the preparation process — unclean hands, improperly stored powder, or contamination anywhere in the mixing/feeding chain can introduce bacteria, so strict hygiene throughout preparation and storage is essential. Correct preparation matters. Use only the scoop provided with the tin, levelled correctly (not heaped, not under-filled) — never a household spoon. One scoop mixes with 30 ml (1 oz) of water; two scoops with 60 ml; three with 90 ml, and so on. Always measure the water first, then add the powder — adding powder first and then filling to a mark gives less actual water than intended (e.g. 90 ml worth of powder in only 75 ml of water), over-concentrating the feed. Don't shake vigorously, as this creates bubbles that cause gas when swallowed. Never improvise the ratio (e.g. using 2.5 scoops because the baby only drinks 75 ml) — over-concentration causes constipation and excess sodium load; over-dilution under-delivers calories and energy. How much and how often to feed. Feed according to the baby's demand/appetite. A rough guideline is about 150 ml per kg of body weight total milk requirement per day (breast or formula combined), but actual intake varies feed to feed, just as adults eat different amounts at different meals — there's no fixed number; feed enough to satisfy the baby and support healthy weight gain. Wherever possible, offer breast milk first (direct or expressed), and only give formula for the remaining need. Does formula cause a chubbier baby? First, a chubby baby is not necessarily a healthier baby — this common belief (often reinforced by grandparents and relatives) should be let go of; what matters is that weight is age-appropriate per the growth chart, and the child is active and growing well. That said, it is observed in some babies that formula-fed infants gain a bit more weight, partly because bottle feeding requires less sucking effort than breastfeeding, and babies sometimes drink more via bottle. This isn't universal across all babies. Try to maximise breast milk supply (a separate video on increasing milk supply is referenced) and use formula only for the gap, phasing it out as supply improves. Why might a baby suddenly reject formula? Not necessarily any fault of the mother — usually a lack of information about the cause. Common reasons: nipple confusion if switching from direct breastfeeding to bottle feeding, since bottle sucking mechanics differ from the breast; a change in formula brand/type changing the taste; occasionally an oral fungal infection (candida/oral thrush) causing mouth pain that makes feeding uncomfortable; or an unsuitable bottle (paladai/spoon feeding is generally preferred over bottles in the doctor's practice). Water for formula preparation. Water must be boiled for children under 2. Practical tip: boil 1 litre of water in the morning (boiling for 5 minutes once it reaches a rolling boil), store it in a good quality flask/thermos, which can maintain the recommended 70°C for about half a day; re-boil and refill as needed rather than boiling fresh water every single time. Opened tin shelf life. Generally one month after opening, if kept airtight in a cool, dark place — no refrigeration needed, but moisture and air must be kept out.
