Cow's Milk for Babies: Right Age, Quantity, Full Cream vs Toned & How to Start Safely

A pediatrician answers common questions from mothers about cow's milk for babies — when to start it, whether it can replace mother's milk if supply is low, how much to give per day, which type (full cream vs toned) to choose, and how to introduce it safely. Covered in this video: When cow's milk can be started 0–6 months: only mother's milk is recommended; if unavailable, formula milk is used. Cow's milk cannot be given at all as a milk replacement in this period. 6–12 months: mother's milk remains best; formula can be used if breast milk isn't available. Cow's milk alone, as a milk replacement, still cannot be given at 6–12 months. However, cow's milk CAN be used while cooking cereals — e.g. dalia, oats, rice, poha cooked with milk into a kheer or porridge-like preparation — this is allowed. Cow's milk alone as a drink/milk replacement is not allowed until 1 year of age. After 1 year, direct cow's milk can be given. Why cow's milk isn't given alone before 1 year The protein in cow's milk is complex, and a baby's intestines aren't ready to digest it fully until 1 year, causing digestive problems. Cow's milk is deficient in iron and vitamin C — repeated use as a milk replacement can cause iron and vitamin C deficiency. High sodium and protein content can stress a baby's kidneys. Can cause cow milk protein allergy, leading to poor food absorption, diarrhoea and loose motions. When cow's milk is cooked with cereals for 6–12 month olds, the starch changes the composition and cooking makes the mixture digestible — that's why it's allowed in that form but not as a standalone drink. Quantity for babies over 1 year 1–2 years: 400–500 ml/day if not breastfeeding or on formula; if still partially breastfeeding, reduce to 300–400 ml so the combined total (breast milk + cow's milk) is around 500 ml. Over 2 years: about 480–600 ml/day. How to start cow's milk Start with a small amount, 50–100 ml, and watch for any reaction — skin rashes, loose motions, diarrhoea, constipation or vomiting — before gradually increasing to the full recommended quantity. If the baby has trouble adjusting, dilute cow's milk 1:1 with clean boiled water (or with mother's/formula milk) for the first one to two weeks to ease digestion, then gradually reduce dilution back to full-strength milk — dilution reduces calorie density, so it's only for short-term adjustment. Health drinks (Horlicks, Complan, Boost, etc.) These are generally labelled for children over 2 years and, while they add flavour and some nutrition, are not the doctor's recommendation. Instead, he suggests a homemade dry-roasted nut powder (almond, pistachio, groundnut, walnut) mixed into milk or into cooked cereal preparations like kheer or sevai — more nutritious than commercial health drinks, which contain a lot of preservatives. Benefits of cow's milk for children over 1 year Important source of calcium and protein, both essential for bone growth. A key protein source for vegetarians. Rich in B-complex vitamins, beneficial for brain growth and overall immunity. Packaged cow's milk today is also often fortified with vitamin D. Which cow's milk to choose Full cream cow's milk is recommended for children up to 2 years, and generally for children over 2 as well, since it contains fat, essential fatty acids, and fat-soluble vitamins A and D — important for growth, calorie needs and brain development. Toned/skim milk is only suggested if a doctor specifically recommends it due to a child being overweight or obese. Good branded packaged cow's milk or fresh milk from a local milkman are both fine, but always boil it thoroughly before giving it to kill bacteria.
